Effect of Fasd (Venesection) in the Management of Acute Cases of Irqunnasa (Sciatica): A Case Series Study

Abstract


Unani scholars described all the painful conditions of the joint under one caption i.e., Waja-ul-Mafasil, which, had later been given the name according to the site of the pain. The pain which starts from the hip joint and radiate towards the lower extremity is called as Irqunnasa (Sciatica). In fact, inflammation of sciatic nerve is called Irqunnasa. The classical presentation is pain and tenderness along the course of the sciatic nerve. According to the Unani classics accumulation of Ghaleez Khilt-e-Dam or Khilt-e-Balgham Ghaleez in the hip joint is the most common cause for Irqunnasa. Tanqiya (Evacuation) of this accumulated morbid matter is necessary in order to manage the disease. Fasd (venesection) is one of the modalities which is used for the purpose of Tanqiya-e-Mawad (Evacuation of Morbid Matter) and is considered as a first line of treatment in managing the acute symptoms of Irqunnasa. Keeping this in view, three cases was selected for the evaluation of the effect of Fasd in the management of the acute symptoms of Irqunnasa on the basis of exclusion and inclusion criteria.  Fasd was carried out at saphenous vein around the medial malleolus of affected limb on the 1st day and 5th day for one week only. Assessment of the patients before and after the intervention was carried out on the basis of following objective parameters like visual analog scale (VAS), oswestry disability index (ODI) and straight leg raising test (SLRT). It was observed that there was a significant improvement in the symptoms, objective parameters as well as in the general condition of patients. No adverse effects were observed during the study. On the basis of the above results, it can be concluded that the Fasd therapy may be used as an effective mode of treatment for the management of Irqunnasa.
